REGISTRATION INFORMATION

PLEASE READ PRIOR TO ATTEMPTING TO REGISTER
Course information:
Sept. 16, 17, 18, 23,24,25, 2008 6 – 9 PM each night
Olympic Heights High School
Fee $100.00 LESS $50.00 FOR THE FIRST 20 PEOPLE
MUST ATTEND ALL CLASSES!

PLEASE NOTE: WE HAVE SECURED FOR 1/2 OF THE COURSE FEE FOR THE 1ST 20 REGISTRANTS

CHECK AMOUNT WILL NOW BE $50.00. WE WILL NOTIFY COURSE ATTENDEES, ON THE FIRST NIGHT OF THE CLASS IF THEY RECEIVED THIS ASSISTANCE. IF THEY DID NOT, THEY MUST BRING A $50.00 CHECK TO THE LAST CLASS IN ORDER TO RECEIVE THEIR CERTIFICATION WHEN THEY PASS THE FINAL TEST.

3- AGREE TO STATEMENT
4- SELECT “REFEREE GRADE 8”
5- SELECT COURSE # A5-10-0908-08
6- FILL OUT REGISTRATION FORM
7- PRINT CONFIRMATION PAGE
